Joslin Diabetes Center and the Dimock Community Health Center together sponsor the Elma Lewis Program for Diabetes Care and Education. The specialty clinic—which is located in the Dimock Community Health Center in Roxbury, Mass—provides both one-on-one visits and group education classes to members of the local community (including Roxbury, Mattapan, Dorchester and Jamaica Plain).
Hours: The Weekly Diabetes Education Clinic is open every Friday from 8:30 a.m. to 4:00 p.m. in the Education Resource Center, located on the ground floor of the Richards building.
The Monthly Diabetes Specialty Clinic is open the last Friday of each month from 9:00 a.m. to 4:00 p.m. in the Adult Medicine Department, located on the 2nd floor of the same building. |

Joslin is involved with Joseph Smith CHC’s Live and Learn Project, a nurse-managed program that helps participants reduce the risk of heart disease and manage their diabetes. Participants receive an individual treatment plan that includes lifestyle modification and education to prevent the complications associated with their diagnoses.

Eligibility Requirements for the Live and Learn Project:
If you are a patient of Joseph Smith CHC and have been diagnosed with any of the following health conditions, you are eligible to participate:
- Diabetes
- High Blood Pressure
- High Cholesterol
- Family History of heart disease
- Obesity
- Smoking
