Providence Hospital is taking a very innovative approach to diabetes across the continuum in affiliation with Joslin Diabetes Center in Boston.  In this position you will play a critical role in the start-up of an outpatient Joslin Diabetes Center as well as an inpatient diabetes initiative.  You will be supported by a team of educators and support staff as well as a seasoned administrator.

Other team members will be involved in community initiatives under a CMMI grant.  A select team of PCPs will be trained and certified by Joslin based on their excellent care of diabetes patients as part of the overall plan to improve diabetes care throughout the region.

Affiliate at Upstate Medical University

Position: RN, CDE

RN NYS license wth 3 years clinical experience (preferably experience with diabetes care and management). Excellent written/oral communication skills and strong computer skills required. BSN preferred. CDE preferred. If not currently a CDE, must obtain within 3 years of hire. 

The primary focus of this position is to provide diabetes education to patients at the Joslin Diabetes Center, Upstate University Hospital (UUH) and ensure that the outpatient diabetes program meets the patient education standards of the Joslin Diabetes Center and UUH. This position will also interact with other staff members and/or providers in a consultative fashion to assist in the provision of diabetes education. Major duties and responsibilities include: provide education in diabetes self management to patients and their families newly diagnosed with diabetes and those patients managing their diabetes for years; develop and implement an individualized education plan based on a needs assessment; work closely with the multi-disciplinary UUH staff as well as the Boston Joslin Diabetes Center to develop policies and protocols pertaining to diabetes; manage urgent sick day episodes of care via telephonic triage; conduct staff as well as community education; maintain accurate medical record documentation and other duties as assigned.

Position Summary: The Registered Nurse, Diabetes Educator is a member of the interdisciplinary team working in the out-patient diabetes education program.  The RN utilizes the care process consistent with their license or certification including initiation of interventions within individual scope of practice.  The RN incorporates diabetes education principles that are current and reflective of the standards set forth by the American Diabetes Association, The American Association of Diabetes Educators and The American College of Endocrinologists in respect to the management of Type 1, Type 2 and gestational diabetes.
By participating in educational activities, the RN assumes responsibility for continued professional and personal growth.
 

Qualifications:Graduate of an accredited school of nursing required. BSN preferred.

Education: Bachelor’s degree in related field required.  Master’s degree is preferred.

License/ Certification: Current license as a registered Nurse in the State of New Jersey. Certification from the American Association of Diabetes Educators as a Certified Diabetic Educator (CDE) preferred. BLSHCP required.

Experience:  Minimum 3 years experience in caring for people with diabetes. Excellent verbal and written communication skills, as well as working as a member of a team. Excellent customer service skills.
